Meet Victoria
*Maternity Management Participant
Victoria found out she was 7 weeks pregnant with her second child. Though she was excited to discover the news, she was also nervous because of past pregnancy complications and history of gestational diabetes.
Victoria knew WebTPA offered maternity management services, so she reached out to the care team for advice. She met the program qualifications and was assigned a maternity management nurse who coordinated monthly check-in calls to answer Victoria’s questions and observe her health.
When Victoria was diagnosed with gestational diabetes at 28 weeks, her maternity management nurse immediately provided education resources regarding diet, glucose monitoring, and symptoms to watch for. They maintained constant communication throughout the third trimester.
Though Victoria and her baby were at high risk for infection and an extended hospital stay after delivery was likely, they experienced no complications and returned home after only one extra inpatient hospital day for observation.
With support from WebTPA, Victoria had an advocate to lean on throughout her pregnancy journey. These interventions also drastically reduced Victoria’s health bills in the coming months.

Meet Linda
*Chronic Condition Management Member
Linda struggled with her health throughout her adult life. She was eventually diagnosed with cardiomyopathy in addition to several chronic conditions: heart failure, low thyroid disease, high blood pressure, and high cholesterol.
Though Linda had discussed cardiac rehab options with her cardiologist, the decision was made that she should attempt healthy lifestyle modifications first. A care management nurse, experienced in chronic conditions, connected with Linda and together they reviewed relevant health education, set realistic health goals, and even completed a course regarding living with heart failure through the American Heart Association.
In the following months, with support from WebTPA, Linda successfully lost 59 pounds after remaining consistent with her diet, exercise, and medications. Moreover, she remained positive and motivated to continue towards her health goals. These improvements prevented a need for hospital readmission.

Meet Sam
*Behavioral Health Program Member
Sam struggled with addiction throughout his late adolescent years. Eventually, he was hospitalized on a voluntary basis for behavioral health and substance abuse.
During Sam’s hospital stay, a behavioral health management nurse coordinated with his family to ensure he had a safe home environment after being discharged. Due to unforeseen circumstances, Sam’s discharge plan changed as he needed to live with an extended family member in another city.
His behavioral health management nurse worked diligently with Sam’s extended family to ensure a smooth transition to this new city. She helped him establish care with a new primary care provider and found a Wellness Center that offered daily, intensive outpatient treatment with group counseling, individual therapy, psychiatric oversight, and medication management.
With support from WebTPA, Sam’s discharge was not delayed, and he was enrolled in an exceptional outpatient program that provided him with the resources he needed.
